25-26 Student Device Insurance

Hello Parents/Guardians of Grades 3rd thru 8th students,

Rio School District participates in a program that provides students with a laptop computer that they can take home and use for their schoolwork, when needed.

The district offers insurance for your student’s device. The insurance provides protection for your student’s device at an affordable cost.

There is only one price and plan offered.

You can pay online (below) or payments can be mailed, or dropped off at the Rio School District Office, 1800 Solar Drive, Oxnard, CA 93030. The district will mail out confirmation of receipt after the money has been processed.

Please review and fill out attached form. You can also request a paper form from your school office.

Insurance Plan Cost: $20.00 No Deductible

Plan coverage: Damage, Loss, and Theft with unlimited claims, full transferability

I am choosing to purchase insurance for my student’s device. I understand that it is my responsibility to sign-up and pay the $20 dollars directly to the Rio School District. Make checks payable to Rio School District.

NO INSURANCE PLAN

If you are choosing NOT to purchase insurance for your student’s device. You understand that you may be responsible for the device if it is damaged or lost. You understand that the value of the laptop, case and charger can cost up to $360 and you may be responsible for the cost of repairing or replacing the device of this amount.
